CDC Communities Putting Prevention to Work: Increase Access to Cessation Services (Santa Clara County, CA)

The goal is to increase cessation within communities with the highest tobacco use rates through culturally appropriate services provided through community-based organization partners.
The intervention has resulted in a 39% quit success rate.

The goal of this program is to provide timely, effective, clinically appropriate intervention for abnormal Pap tests at no or very-low cost in order to reduce cervical cancer mortality.

The goal of Check & Connect is to encourage middle and high school students to stay on track towards graduation. Check & Connect seeks to foster student engagement at school and with learning. In Check & Connect, engagement is defined as commitment to and investment in learning, as well as identification with and belonging at school. Engagement is associated with desired academic, behavioral, cognitive, and affective outcomes, such as persisting in school and graduating.
Students who participate in Check & Connect are significantly more likely to stay in high school than similar peers who did not receive the intervention (24%). Students who participate in Check & Connect are significantly more likely to complete high school than peers who did not receive the intervention (20%).

The goal of this workshop is to build the participants’ self-confidence in their ability to manage their health and maintain active and fulfilling lives despite their chronic disease condition.
In 2016 Derby Health Collaborative hosted six free self-management education courses in partnership with the Derby School District, the Derby Senior Center, the Derby Public Library and the Kansas State Extension and Research Center. Workshops will continue in 2017.

The goal of the Colorectal Web interactive website is to promote colorectal cancer screening.
Colorectal Web is more effective than a standard colorectal cancer website at prompting previously unscreened individuals to choose a preferred colorectal cancer screening test and to be screened for colorectal cancer.

The goals of this promising practice were to identify the transportation-disadvantaged population that lacks nonemergency medical care because of low access to transportation; determine the medical conditions that this population experiences and describe other characteristics of these individuals, including geography; estimate the cost of providing the transportation necessary for this population to obtain medical transportation according to various transportation service needs and trip modes; estimate the healthcare costs and benefits that would result if these individuals obtained transportation to non-emergency medical care for key healthcare conditions prevalent for this population; and compare the relative costs (from transportation and routine healthcare) and benefits (such as improved quality of life and better managed care, leading to less emergency care) to determine the cost-effectiveness of providing transportation for selected conditions.
These results show that adding relatively small transportation costs do not make a disease-specific, otherwise cost-effective environment non-cost-effective. Providing increased access to non-emergency medical care does improve quality of life and saves money per patient.

Cradle Kansas City only has one goal, to close the health equity gap. By doing this, they will impact premature birth and infant and maternal mortality. They accomplish this through partnerships medical systems, resident-built strategies, and clear messaging that is aimed at systemic change.
